Primary Duties and Responsibilities
o Coordinate medical records for clinical nutrition. Ensure medical records (EHR) reflect nutritional status of the individuals and meet regulations. o Completion of nutritional assessment assessments using Nutrition Care Process and discharge plans. (The frequency is dependent upon clinical needs and regulatory statutes - generally annual and quarterly reporting and at discharge.) o Review menus for regular and modified diets. Recommend changes to meal plans to address nutritional concerns. o Attends Comprehensive Functional Assessments (CFA), Special Case Reviews, Admission/Transfer and Discharge Meetings. o Provide Medical Nutrition Therapy (MNT) and make clinical recommendations to the treatment team for the maintenance and/or improvement of nutritional status. o Work as part of an interdisciplinary team to assess feeding and swallowing concerns. o Schedule and conduct Nutrition in-service education/training for house staff. Maintain attendance records and record topics covered. o Provides nutrition education to individuals as needed and periodically assists R.N. with health class. o Maintains collaborative relationships with the food service department. o Maintains collaborative relationships with other clinical disciplines (SLP, OT. PT, nursing) to provide comprehensive service delivery to individuals. o Participate in New Employee Orientation (NEO) training on a rotating basis through Staff Development and Training at the BDC location o Participate in the development and implementation of plans for corrective action. o Ensure OPWDD regulations are adhered to and for overall nutritional health and safety of the individuals. o Regular and proficient computer use of clinical documents inclusive of the Electronic Health Record (EHR) of everyone within the clinical caseload. o Conduct regular Nutrition and Sanitation Oversight Reports (NSOR) for residential sites within assigned caseload. (Completing NSOR requires 1X/week working past usual work schedule. Details of this will be provided at the interview.) o Assist in monitoring food expenditures/budgets. o Participate in program/service planning meetings (Life Plans), house specific meetings and other meetings related to operational needs of the Agency. o Participate in State Operations, Region 2 Nutrition Services Discipline Meetings. o Participates as a member of the governance committee- attending the weekly meeting. o Completes and remains current with Agency/State required annual trainings (i.e. fire safety, SMLS trainings, Praise, and Promote Training, etc.) o Other duties and caseload assignments as assigned. o Frequent travel between assigned ICF's is expected. Involves use of a state vehicle, if a vehicle is unavailable mileage is compensated per Office of State Comptroller rules and regulations

Salary Range:
From $61270 to $77912 Annually
Minimum Qualification
MINIMUM
Qualifications:
Must currently be in title, transfer eligible, or active and reachable on the Civil Service Eligible List. Should the canvass result in less than 3 acceptors, candidates meeting the open competitive Civil Service minimum qualifications will be considered. PROVISIONAL
Qualifications:
You must have your current registration with the Commission on Dietetic Registration (CDR) as a Registered Dietitian (RD). You must indicate your RD registration number on your application. OR You must have your New York State Education Department (NYSED) certification in dietetics-nutrition AND have completed a minimum of an additional 400 hours of supervised dietetics practice under the supervision of a RD. You must indicate your NYSED certification in dietetics-nutrition number on your application. If you are qualifying under #2, the additional 400 hours required is in addition to the 800 hours of supervised dietetics practice under the supervision of an RD required for NYSED certification. On your application you must provide the name of the institution where you obtained the additional hours; the name of your supervisor; and your supervisor's qualifications. Candidates must maintain current certification in New York State as a Certified Dietitian/Nutritionist.Estimated Salary: $20 to $28 per hour based on qualifications.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
