POSITION DESCRIPTION:
This is a full-time position as the Day Program Coordinator at the Norwich Day Hab. The assignment includes providing support in service delivery and supervision of day-to-day operations at the Day Habilitation program.
Primary Duties and Responsibilities
Function in the role of a Rehab Counselor 2 at the Norwich Day Hab, which will include a full range of activities and supervisory responsibilities requiring independence in handling complex issues and assisting in developing habilitation and service programs. Examples of routine duties include:
o Coordinate person-centered service plans with recipients to ensure services provided meet the needs of individuals receiving day services. o Conduct comprehensive evaluations on the habilitation needs of individuals. o Prepare and monitor plans for individuals based on initial assessment and progress. o Collaborate with other members of treatment teams to enhance habilitation services and reinforcement of positive behaviors. o Prepare goals and objectives. o Follow up on issues associated with participation and progress. o Complete reports on participation and progress. o Assist individuals in linking up with and maintaining contact with the various community supports necessary to an inclusive life. o Match and assign individuals to appropriate daily program activities. o Monitor tracking systems. o Assure records are maintained as outlined. o Review policies and procedures with staff. o Coordinate development of goals and programs with subordinate staff and treatment teams. o Participate with the Treatment Team Leader in policy meetings, long-term program planning, and review meetings to update, improve, and maintain programs which meet individual needs and use current evidenced-based practices. o Provide treatment teams with objective information concerning individual's performance abilities in the areas of residential, social, and daily living skills. o Interpret, report, and document all ongoing progress. o Assist with interpreting their daily activities and experiences and the impact upon the habilitation process. o Complete incident reporting and conduct investigations. o Document findings and recommendations to ensure treatment team members receive accurate and timely information regarding needs. o Monitor progress, consult with treatment team, and modify individual plans as indicated. o Participate in treatment team meetings by making recommendations and providing input, and by recommending and implementing modification of plans as required to achieve desired outcome. o Supervise, assign, evaluate Day Hab staff. o Monitor physical plant at Day Hab, reporting issues to work control/maintenance as needed. o Conduct and document fire drills quarterly. o Monitor billing practices. o Participate with DQI/QCR audits as needed. Qualifications:
QIDP standards preferred. The Department of Developmental Services (DDS) adopted the federal definition for Qualified Intellectual Disabilities Professional (QIDP) in all ICF/DD, ICF/DD-H, ICF/DD-N and DD-CNC facilities when the Federal ICF/IID regulations became effective in October 1988. The federal criteria outdate the state regulations specified in the California Code of Regulations, Title 22. QIDP STANDARDS ARE:
o QIDP is a staff who is responsible for integrating, coordinating and monitoring each client's active treatment and who possesses:
o (a) At least one year of experience working directly with persons with intellectual disabilities or other developmental disabilities. o (b) Is either a physician and surgeon, or an osteopathy physician and surgeon, a registered nurse, or a human services professional possessing at least a bachelor's degree in

Salary Range:
From $63323 to $80429 Annually

Qualifications:
o Candidates must be current in title, o Eligible for 70.1 transfer, or o Reachable on the NYS Civil Service Eligible List for this title. Provisional appointment may be considered. For provisional appointment you must meet the minimum qualifications for the Paraprofessional Exam and have the qualifying experience for appointment from the Eligible List. These qualifications are:
be a qualified employee of New York State and have had one year of permanent competitive or 55-b/55-c service as a Direct Support Assistant, Direct Support Assistant (IRM), Direct Support Assistant (Manual Communications) formerly known as Developmental Aide, Developmental Aide (IRM), Developmental Aide (Manual Communications) , Developmental Disabilities Secure Care Treatment Aide 1, Licensed Practical Nurse, Mental Health Therapy Aide, Secure Care Treatment Aide, Teaching Assistant, Training Aide, or in a language parenthetic in any of the above titles.Estimated Salary: $20 to $28 per hour based on qualifications.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
